Please indicate your acceptance of the following Terms and Conditions by checking the box below: I give my express consent to the public posting of my photo on the www.filminflorida.com website, and to being publicly identified by name if my photo is used for “Hot Pics.” The photo that I am submitting is my original work and does not infringe on anyone else's rights. By submitting the photo I am giving the Governor’s Office of Film and Entertainment a perpetual, royalty-free, worldwide license to use the photo for its business purposes, including “Hot Pics”. I hereby give to the Florida Governor’s Office of Film and Entertainment, and all persons, partners and corporations acting with its permission (collectively “OFE”), unrestricted permission to copyright, use, publish, and broadcast the image or likeness of the subject in still, single, multiple, moving, video format, Internet usage, or any other medium of communication. The image and/or likeness of the Subject may be used in whole, in part, in composite, and in distorted form for any lawful purpose. I hereby waive in perpetuity all rights to inspect and approve the finished product that may be used in connection to the image and/or likeness of the Subject, and the use to which the image, voice and/or likeness of the Subject may be applied. I further release OFE from any claims for remuneration associated with any form of personal injury or property damage, foreseen or unforeseen, associated with the commercial, public interest, and artistic use of the image, and/or likeness of the Subject. I acknowledge that the image and/or likeness of the Subject and any resulting use of that image by OFE, shall be the sole property of OFE, which may copyright said property in its own name and with full right of disposition in any manner whatsoever. I understand and acknowledge that the Governor’s Office of Film & Entertainment chooses the “Hot Pics” in its sole and absolute discretion and all decisions made by the Governor’s Office of Film & Entertainment are final. I also understand and acknowledge that by submitting a photo the Governor’s Office of Film & Entertainment does not guarantee acceptance or publication of the photo. I am at least eighteen (18) years of age. I certify that I have read and understand these terms and conditions and give my consent freely to the terms and conditions contained herein
